Physical Examination

Athletes must have a current sports qualifying physical exam on file in the Athletic Office. Physicals must be dated after May 1st of the current school year (ie-physical date May 5, 2022 for the 2022-23 school year) for an athlete to be eligible for participation.

Once you have your physical completed by a doctor, you can turn it into the Athletic office or you can scan it and submit the completed Physical Forms online.

As part of requirements, please complete the OSSAA Eligibility Record Form which can be downloaded on step 6 of the registration. After filling it out, you may upload it or hand it over to the office.
